Insurance & Payments

Everyone deserves affordable health care. If you're concerned about cost, you may qualify for low- or no-cost services. With or without insurance, you can always come to us for health care.

I don’t have insurance or Medicaid

If you don't have insurance, you may qualify for a state-funded program or a lower fee scale. If you’re worried about cost, ask us about how you can get health care that fits your budget.

Our fees are based on your household income. Please bring either a pay stub or unemployment documentation if you have it. We’ll never turn you away from the care you need because of your inability to pay.

Other Options

If you do not have health insurance, read about health insurance options to find out how to get more affordable coverage and what to consider when choosing a plan.

I have insurance or Medicaid

We take the following insurance:

  • Coverage varies by provider

We accept many private and public insurance plans. If you don't see your insurance listed, please call us at 801-479-7721.

Services Coverage

Most health insurance plans now cover prescription birth control, annual wellness exams, and STI/HIV screenings with no copay, and many other services with some copay required. Contact your health insurance company directly to make sure that the services you need are covered and what, if any, out of pocket costs you might have.

Additional Payment Information

Payment is expected at time of service unless other arrangements have been made. We accept:

  • Cash
  • Check
  • Major Credit/Debit Cards
  • HSA/FSA debit cards
  • Money Orders

Private insurance coverage varies by location. Our providers are in-network with many insurance plans. Please call the health center of your choice to check on coverage. Bring your insurance card and photo ID with you to your appointment, and we'll bill them if we can. If your insurance is out of network, we're happy to bill you on our sliding fee scale.
